Recording from raw scores (online entry)

Levels 1 & 2 (grades 1-3, sometimes 4) use a consumable student booklet, which is hand-scored by your teachers. The test information and raw scores need to be submitted to us in order to be scored.

You can enter student, test, and raw scores information for Level 1&2 students through our online portal at CatformsRecords.com. In order to use this, you must purchase the one-time setup fee, which includes a basic, non-expiring subscription.
